For equipment loaned to Employees and Students, please fill out the Equipment Loan Agreement form and email it to mntl-facilities@illinois.edu.
To set up fabricated account code for a project you are building, please fill out the Fabrication Form and email it to mntl-facilities@illinois.edu. When the form is received, facilities will be in touch as soon as possible.
